Registering with the surgery
To register with the surgery:
- fill out an adult new patient registration form online if you are 16 years old or over
- fill out a child new patient registration form online if you are under 16 years old
- fill out a register with a GP surgery form online
- visit reception from 10am to 6pm and collect a paper registration form
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
Once you have submitted your registration form, the surgery will reply via email. You should receive an email within 3 working days. If you have not had a reply please check your junk mail.
You will then need to come into the practice with one proof of ID and one proof of address dated within 3 months to complete your registration as well as to complete GMS1 form and practice policy on the supply of drugs liable to be misused form.
